2026 Sentencing Forms Guidance and Filing Instructions

The Kansas Sentencing Commission is finalizing the integration of the KSApp with the Office of Judicial Administration’s eFlex system. Full functionality is expected soon after OJA approval.
Until then, please follow the guidance below for the Presentence Investigation Report (PSI), Journal Entry of Judgment (JE), and Journal Entry of Probation Violation Hearing (PVJE) forms.

1. Presentence Investigation Report (PSI)

The 2026 PSI form must be used for all felony cases in which the date of offense occurs on or after July 1, 2026, through June 30, 2027.

• The 2026 PSI is currently being loaded into the KSApp for use.
• The 2026 PSI will only be accessed in the KSApp and will not be available on the KSSC website.
• 2026 PSIs should be prepared directly within the KSApp when the form becomes available.
• Pre-2016 PSIs or other filing exceptions should follow the approval process listed below.

2. Journal Entry of Judgment (JE)

The 2026 JE form must be used for all felony cases in which the date of offense occurs on or after July 1, 2026, through June 30, 2027.

• The 2026 JE is currently being loaded into the KSApp for use.
• The 2026 JE will only be accessed in the KSApp and will not be posted on the KSSC website.
• 2026 JEs should be completed directly within the KSApp when the form becomes available.
• Pre-2026 JE forms will also be available, and KSSC approval will be required to use them if the original PSI has not originated in the KSApp. The approval process is listed below.

Be aware that if the KSSC has not approved the JE form through this process, the clerk’s office will reject the filing. 

3. Journal Entry of Probation Violation Hearing (PVJE)

The 2026 PVJE form must be used immediately for all cases in which a hearing occurs on or after July 1, 2026, through June 30, 2027, regardless of the date of offense.

• If the 2026 PVJE cannot be filed through the KSApp, contact the KSSC. The approval process is listed below.

• The 2026 PVJE is only available by contacting the KSSC and will not be posted on the KSSC website.

Be aware that if the KSSC has not approved the JE form through this process, the clerk’s office will reject the filing.

KSSC Form Approval Process for Cases with KSApp Filing Exceptions

The KSSC form approval process for PSI/JE/PVJE is listed below.

Step 1 - Email kssc.ksapp@ks.gov, including form and case number in the subject line (e.g., JE Approval Requested).

Step 2 - Include the case caption, county, case number, and the exception for each requested form (see the KSApp Filing Exceptions Practitioner Quick Guide, options 1-8), along with your name. You may send multiple requests in a single email, with one line per case. If you would like the forms sent to a different email address, please note it.

Step 3 - KSSC will log the request and email the fillable form(s) to the email you specify. The check box titled "Form Approved by KSSC" will be checked by KSSC staff, and the specific case number requested will appear on each form.

Step 4 - Complete the fillable form and distribute it to opposing counsel for approval and signature.

Step 5 - Convert to PDF and file outside KSApp through eFlex.

Be aware that if the KSSC has not approved the form through this process, the clerk’s office will reject the filing.
